Parent/Guardian Camp Evaluation
By filling out this survey you will be entered into a draw to win a free week at Camp Shalom during the 2011 camping season. The winner will be announced on September 18th, 2010 at the Fun Fair and will be contacted after that. Thank you for your input! This survey is for Adventure camp, Adventure Day Camp, Munchkins, Munchkins Reloaded, Trek Unleashed, Algonquin Canoe Trip and Quest only. For the special support evaluation click here.
Terms of contest
The draw will be done on September 16th and the winner will be announced at the Fun Fair on September 18th. The winner may redeem the free week at camp for Adventure camp, Adventure Day Camp, Munchkins, Munchkins Reloaded, Trek Unleashed, Algonquin Canoe Trip and Quest during the 2011 camping season. The contest does not apply to Special Support, Spec Trek or Special support day camp. You may only fill out the evaluation once. If you fill out the evaluation more than once you will be disqualified from the contest. The form must be fully completed to qualify.
